Injury Reporting Form

Information on sports injuries is essential for identifying the spectrum of injuries that occur in different sports and activities. Through a process of injury surveillance, records are kept on items such as the incidence and severity of injuries, mechanisms of injury and initial treatment. This information is used to identify injury problems for subsequent prevention programs. The success of the intervention can also be assessed with ongoing data collection.
